Hiring a Home Health Nurse Practitioner (NP) in the Montreat, North Carolina (NC) area for in-home health assessments of Medicare Advantage members within the geographic area. At least one year of experience as a Nurse Practitioner is required; Interested applicants should apply today!

Responsibilities of the Home Health Nurse Practitioner (NP):

  • Conduct in home assessments on health plan members including past medical history, review of symptoms, physical examination, medication review, depression screening, responsible for checking vitals, conducting a physical exam that includes monofilament tests, urine dipstick, and foot exam as appropriate
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the finding of your assessment
  • Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care
  • Educate members on topics such as disease process, medication, and compliance

Education and Qualifications of the Home Health Nurse Practitioner (NP):

  • Active North Carolina Nurse Practitioner (NP) un-encumbered License required
  • Nurse Practitioner Certification from either the ANCC/AANP
  • Minimum of one year of clinical experience as a NP
  • Ability to obtain DEA licensure/Prescriptive Authority post hire in states where applicable
  • Home Health Experience heavily preferred

Employment Type: Full-time or Part-Time, Contract

Work Hours: Flexible Visits are on average 45-60 minutes; Must commit to a minimum number of visits per week and follow through on this commitment

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
